Have Direct Access to
Physical Therapy.
No referral is needed for physical therapy.
Insurance
Performa is an out-of-network provider. The most common plans that have out-of-network benefits are employer-sponsored plans and those bought through Covered California.
Payments
You can use your Health Savings Account (HSA) and Flexible Spending Account (FSA) and easily pay with your personal credit card.
We can provide a superbill to submit for reimbursement to the insurance company or HSA/FSA account manager.
